Request To Be Appointed Forms

There are two different forms to request appointments to a BVSA Advisory Committee.  First is a request for individuals to be appointed to a committee during the current term of service. The second is the annual request for all individuals who wish to serve during the upcoming term.  All committee appointments must be approved by the BVSA Board of Directors.  Requests to be appointed to a committee should be submitted to the Board Secretary, Karen Luginbuhl no later than the Monday preceeding the Board meeting.  The Board meets the 3rd Saturday of each month.  To submit the request, you may drop it off at the Association office, or email it to the Board Secretary at karenl.bvsa@bluetie.com.  The downloadable request forms are posted below.
